MPORTANT HEALTH COVERAGE TAX DOCUMENTS
In accordance with IRS guidance under Sections 6055 and 6056, the Office of Benefits and Retirement has chosen to furnish Forms 1095-C upon request rather than automatically mailing them. These forms provide information about your health coverage for tax purposes.
How to Request Your Form 1095-C
If you would like to receive a copy of your 1095-C, you may request using any of the methods below:
Download: Visit bcps.bswift.com and login with the following credentials.
- Your username is your first initial of first name + last name + your year of birth.
- Your initial password is the last 4 digits of your SSN. You will be required to reset your password.
Once logged in you will see a tile on the home page that reads “Get my ACA form”. Simply click the form to view or download.

Your form will be mailed to you within 30 days of receiving the request
When submitting your written request, please include the following required information:
- Your full name
- Your employer’s name (Baltimore County Public Schools)
- Current mailing address
- Tax year for which you are requesting the form
