Registration Forms and Manuals
- Do we need a new registration
form for each student each school year? Yes, a new registration
form must be completed for each student each school year.
- Who should receive
the registration once they are completed and returned to
school? Once the registration form is completed, please
send it to the Office of Third Party Billing in Timonium.
You may send the form through our inter-office mail.
- We sent in some registration
forms for students whose names did not appear on our September
printout. What should we do? We are still registering
students at the Office of Third Party Billing; therefore,
additional names may appear on your October's printout.
If they do, you may submit encounter data forms retroactive
nine (9) months prior to the child's name appearing on the
listing.
- Who should receive
the registration form? We suggest that you send
a copy of the Third Party Billing registration form
home to the parents of all students enrolled in the
special education program. You may also choose to talk
to parents about the new Maryland Children's Health
Program (MCHP), which will allow several families who
may not have qualified for Medicaid in the past to
qualify under the expanded program. Your school nurse
has enrollment forms for this new program.
- We submitted a registration
form for a student, but his name did not appear on our printout.
Why? There could be several reasons the students name
did not appear on your printout.
- The Medical Assistance number may not have been active
at the time the Office of Third Party Billing called
and verified the status of the social security number.
- The eligible list of students may have been printed
before the registration form arrived at the Office of
Third Party Billing.
- The IEP may not have been active. In this case, you
would need to check the IEP and send the information
to The Office of Student Data so they can update their
records.
