The role of the transition facilitator continues to support each child and family as they transition from middle to high school to post-secondary opportunities. In offering this support, each facilitator provides a network of resources, community contacts, alternative educational programs, linkage to adult services, post-secondary options, team participation, and work opportunities.

IDEA is very clear in its mandate to address all aspects of the student in the transitioning process. Each facilitator begins as early as seventh grade to assess career interests, goals and academic choices. In eleventh and twelfth grade, or two years before exiting at age twenty-one, special emphasis is given to linking, recommending, counseling, and tracking post-secondary goals.
