Section A – Voluntary Transfer Request (Former Preliminary Selection Form)
Completed by receiving school
Submit to Personnel Office prior to Staff Review Meeting
Section B –Internal Personnel Change Request (Examples: FTE – change from 1.0 to .5; Budget – change from Title One to Operating; Subject – change from Math to Science)
Submit to Personnel Office continuously for changes not included on the Organization Worksheet
Section C – New Hire Request (complete for the selection of new hires)
Submit to Personnel Office continuously

Personnel Officers will “forward” confirmations to Area Assistant Superintendent, Receiving Principal, Sending Principal, employee, and Position Management.
