Parent Responsibilities
| Referral:
- As soon as you learn that your child will be absent
from school for at least four weeks, contact your
child's guidance counselor. The counselor will complete
the Referral for Home Teaching.
- Obtain a copy of the Application for Instructional
Program for Homebound or Hospitalized Student from
your child's guidance counselor. Have your physicians
complete the form. You must also sign it, and have
the physician mail or fax it to Home and Hospital
Center. As soon as Home and Hospital Center receives
both referral forms, your child will be scheduled
and contacted promptly by the Home and Hospital Center
personnel.
Chronically Ill Program:
- Obtain the books and work from the regular school
teacher to have available for the home teacher. Chronically
ill students are not generally assigned to telephone
classes.
- Ensure that a supervising adult is present during
the home visit and that the tutor and the student
have an appropriate place for instruction.
Teleclass:
- Monitor your child's attendance in the assigned
classes.
- Monitor assignments that are to be mailed in. Assignments
are posted on SchoolNotes.com.
- Ensure that a supervising adult is present during
home visits that are scheduled for extra help or enrichment.
Also ensure that the tutor and the student have an
appropriate place for instruction.
Home Teaching:
- Ensure that a supervising adult is present during
the home visit and that the tutor and the student
have an appropriate place for instruction.
- Call the Home and Hospital Center or the tutor before
the scheduled home visit if your child is too ill
to meet with the teacher.
Absences:
- Call the Home and Hospital Center (410-887-3222)
before the scheduled class time if your child is unable
to participate in the day's lesson. Excused absences
include sickness, a doctor's visit, and a death in
the family. Try to schedule appointments around your
child's class time.
- For pregnant students, please notify the Home and
Hospital Center when your daughter delivers her baby.
She is entitled to be absent for five school days
after the birth of the baby.
Termination of the Program:
- Obtain a written note from your child's doctor indicating
the date that your child may return to regular school.
This note must be given to the guidance counselor
or school nurse before your child attends any classes.
- Notify the teachers at Home and Hospital Center
that your child will be returning to regular school.
