E-mail Request & Acceptable Use Policy Forms 
Instructions for Completion and Submission
  1. Enter the requested information in the form below and click the 'Generate Printable Form' button to automatically generate printable forms. Acrobat Reader software must be installed to view and print these forms. If you do not have Acrobat Reader, you may install it by clicking here.
  2. Print the form(s).
  3. Sign the Acceptable Use Policy for Employees form and send it via interoffice mail to the AUP Form Processor, Department of Personnel, Greenwood.* If you have already signed the AUP form and are now applying for an e-mail account, you may skip this step.
  4. Initial each of the checkboxes on the Electronic Mail Application form and sign as indicated. After your Office Head/Principal has signed the form, send it via interoffice mail to Postmaster, Department of Technology, Timonium.

It may take up to two weeks to process your form.

*If you were not hired by the Department of Personnel, but were hired by the Office Head or Principal of your work location, please have them file a copy of your AUP form on site.
